Description

In partnership and funding from Project Momentum, ABC2 is excited to host Heart Matters Community Sessions at the Dawson Resiliency Hub.


We are recruiting 24 - 30 participants to engage in a 12 - 18-month program. Monetary Compensation is provided based on participation.


Project goals:


Prevent health problems and promote equity.


Learn more about how to manage your heart health, including heart-healthy behaviors.


Establish a supportive and sustainable local environment to improve heart health.


Heart Matters is an evidence-based program that reduces heart disease risk for African Americans.


ABC2 is hosting a Kick-Off Interest Meeting with interested participants on Friday, August 30, 2024 @ 11am at the Dawson Resiliency Hub in Scotland Neck, NC. 


Participants Must:

  • At least 18 years old
  • Reside in Rural Eastern NC
  • Identify as African American
  • Have at least One Pre-Existing heart-health related condition. (i.e. High Blood Pressure, Diabetes, High Cholesterol, etc)
