Hello and thanks for joining! Due to our transportation capacity we are only able to open up limited spots for our field trip activities. In order to give students an equal opportunity to sign up and join, parents are required to fill out a form for specific activities in order to claim a spot. Once all of the spots are filled, you will be unable to submit a response to this form.

If you have more than one middle school student, you must fill out this form for each student who is planning to attend! 

If this form is full but you want your student(s) to be placed on a waiting list, you can email or call Coda Mix (codam@thecentersb.com or 231-787-2900, ext. 405) to register your student for any spots that may open up.

As a reminder to parents and students alike : IF you sign up for a spot, you are expected to join, barring emergencies. If you need to or want to cancel for any reason, we are asking for 24 hours cancellation notice so we're able to give the spot to someone on our waiting list. Failure to do so will result in your student[s] losing their spot for the next limited-attendance adventure they are signed up for.
